Mental health and substance abuse care are provided by United Behavioral Health. Different levels of benefits are paid depending upon whether you obtain In-Network Services or Out-of-Network Services. To receive the highest benefit level, one must use In-Network Services. To receive the maximum benefit that is payable when using Out-of-Network Services, one must comply with UBH's Certification or Pretreatment Outpatient Assessment requirements.
